Patient Background

Personal Details

Patient Name Mrs. Neha Malhotra (Fictional)
Age 56 Years
Gender Female
Location Janakpuri, West Delhi
Occupation Homemaker
Marital Status Married
Living With Husband (60 yrs) and Daughter

Clinical Context

Mrs. Malhotra had been managing her household in Janakpuri for decades before joint symptoms began affecting her daily routine. As a homemaker, her days involved prolonged standing, repetitive hand movements for cooking and cleaning, and regular outdoor errands. These activities became progressively difficult as her joint symptoms worsened.

Her husband, at 60 years old, had taken on an increasing share of household responsibilities but was not trained in joint protection techniques or safe movement practices. The family decided to seek structured home healthcare support after her rheumatologist recommended regular physiotherapy and closer symptom monitoring.

The family had access to rheumatology centres in West Delhi and Central Delhi, but frequent hospital visits were disruptive and tiring for Mrs. Malhotra, particularly during flare-up episodes when movement itself was painful.

Clinical Diagnosis

Primary Diagnosis

Rheumatoid Arthritis (RA)

Understanding Rheumatoid Arthritis

Rheumatoid Arthritis is a chronic autoimmune condition in which the body’s immune system mistakenly attacks the synovium, the thin lining of the joints. This produces inflammation that, over time, can damage cartilage, bone, and surrounding joint structures.

Unlike osteoarthritis, which results from mechanical wear, RA is systemic. It typically affects joints symmetrically on both sides of the body, most commonly the small joints of the hands, wrists, and feet. It can also involve larger joints and, in some cases, affect other organ systems including the lungs, heart, and eyes.

A hallmark feature of RA is prolonged morning stiffness lasting longer than 30 minutes, which improves with movement. This distinguishes it from the shorter stiffness seen in osteoarthritis.

Clinical Findings in This Patient

  • Persistent joint pain affecting multiple joints
  • Morning stiffness limiting early-day activities
  • Reduced hand strength and grip
  • Difficulty standing for extended periods
  • Fatigue during and after daily activities
  • Progressive difficulty with household tasks
